The type of personal information we collect
We currently collect and process the following information:
• The name, email address and telephone numbers of people expressing their interest in volunteering with their registered GP practice.
How we get the personal information and why we have it
Altogether Better is an NHS team working with GP practices to support them to develop a new way of working that involves collaborating with volunteers (called ‘Practice Health Champions’) from their patient list.
The personal information we process is provided to us directly by you because you have received information from your GP practice (usually via an SMS message from your practice) about volunteering with them and have decided to express an interest in getting involved. You have done this by sharing your details with Altogether Better and your practice via our secure Expression of Interest Portal (hosted by SurveyMonkey).
We use the information that you have given us in order to follow up with you to let you know the next steps for volunteering.
We will share this information (via encrypted NHS mail) with your own GP practice so that they know who has expressed an interest in volunteering with them.
Under the General Data Protection Regulation (GDPR), the lawful basis we rely on for processing this information are Article 6 (1) (e): Public task.
How we store your personal information
Your information is securely stored:
a) On the secure servers of the expression of interest portal owned by Survey Monkey (Momentive). You can find their Privacy Notice here.
b) Once it has been exported from SurveyMonkey it is encrypted and stored on our secure NHS servers.
We keep your contact details for the duration of our work with your GP practice. This is typically between 6-12 months. We will then dispose of your information in line with NHS protocols.
Your data protection rights
Under data protection law, you have rights including:
Your right of access - You have the right to ask us for copies of your personal information.
Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances.
Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.
Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.
Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
