Health Equity Programme

A programme for people working in primary care who are keen to develop the knowledge, skills and resourcefulness to take action to address health inequalities.


About the programme

Offering simple, practical support the programme develop the capability of PCN or practice staff who wish to address particular areas of health inequalities through their work.  Originally designed and delivered for North East London Health Equity Academy, the programme leads to:

  • Delivery of more equitable care

  • Probable reduction in appointments requested (depending on issue addressed)

  • Potential for higher QOF payment

  • An intervention that works will save time repeating interventions that don’t work

Participants have included multidisciplinary teams from individual practices, single PCN teams or multiple PCNs from one locality working together on a shared ambition.

Over the course of five modules or skill development and organisational support the participating teams will:

  • Create the conditions for impactful work

  • Build relationships and shared understanding

  • Design an intervention to tackle a ‘wicked problem’

  • Continuously improve

  • Take steps to share and spread collective learning

How is the programme delivered?

Delivered over 5 months, this programme is made up of 5 half-day workshops. These will be delivered according to the preferences of participating teams and are likely to be a combination of online and face-to-face modules. The programme also includes:

  • Development of a community of practice

  • Support to identify and provide the data you need for the issue you want to tackle

  • Bespoke -2-1 consultancy support between modules for those who need it

How to join

This programme can be commissioned at area level, inviting multiple trusts to send participants to work as part of a wider community of practice, or bespoke programmes can be delivered at trust level.
